Which Correction or Replacement Form do you Need?

IMM 5009

Form IMM 5009 is the application for a Verification of Status (VOS) document or a replacement copy of a valid temporary resident document. If you need to obtain a Verification of Status document or if your valid temporary resident document has been lost stolen or destroyed, you may file form IMM 5009 for a replacement.

IMM 1436

Form IMM 1436 is the request for an Amendment to a Record of Landing, Confirmation of Permanent Residence document, or an amendment to certain Temporary Resident documents. If the IRCC (formerly known as CIC) has made errors on one of these documents, use this form to have them corrected.

Which Correction or Replacement Form do you Need?

IMM 5009

Form IMM 5009 can be used to obtain a Verification of Status document. This document can be used as proof of historical immigration information and will contain information that was included on any of the following immigration documents:

  • IMM 0056, Certificate of Departure
  • IMM 1000, Immigrant Visa and Record of Landing
  • IMM 5292 (or 5688) Confirmation of Permanent Residence
  • IMM 1097 (or 1442), Visitor Record
  • IMM 1102 (or 1442), Work Permit
  • IMM 1208 (or 1442), Study Permit
  • IMM 1214, Exclusion Order
  • IMM 1215, Deportation Order
  • IMM 5238, Departure Order
  • IMM 1263, Permit to Come Into or Remain in Canada
  • IMM 1203, Authorization to Return to Canada

You may also file IMM 5009 to obtain a replacement of a Valid Temporary Resident Document. You may file to have the following documents replaced:

  • Visitor Record
  • Work Permit
  • Study Permit
  • Temporary Resident Permit
  • Permit to Come Into or Remain in Canada

IMM 1436

You may use Form IMM 1436 to correct errors of the following information:

  • Family name
  • Given name(s)
  • Date of birth
  • Country of birth
  • Citizenship
  • Sex
  • Marital status
  • Date of original entry
  • Date of landing

It is important to note that form IMM 1436 can only be used to correct information on your Record of Landing, Confirmation of Permanent Residence, or Temporary Resident document that reflects your personal information at the time the document was originally issued.

  • Study Permit
  • Temporary Resident Permit
