IMM 5009 Verification of Entry

Immigration status documents can help immigrants in a number of ways, primarily with applying for benefits or other immigration benefits.

Generally these documents are granted to the holders upon approval of the benefit or the service. Many of these documents are granted upon an immigrant’s arrival to Canada.

However, during the course of living in Canada it is possible that you have misplaced or lost one or a few of these documents. Citizenship and Immigration Canada can understand this conundrum and offers form IMM 5009 for immigrants to apply for a replacement of these documents.

This form is not to be used for changing the name or information listed on immigration documents. There are different processes for changing your name on an immigration document and for having information corrected by the CIC.
